Grilo et al. (1996) found that female adolescent psychiatric inpatients were more likely than males to meet criteria for BPD. Biased assessment instruments refers to the items used in instruments themselves applying to one sex more than the other. There does not appear to be any literature on this form of bias in relation to instruments for measuring BPD. The more general PD literature supports that there are biases more often on scales that involve disorders more often diagnosed in men (Widiger, 1998b), such as narcissistic and ASPDs.

Leyton et al. (2001) studied 13 medication-free patients with BPD (eight female) using a PET scanning technique that indexes serotonin synthesis capacity, and compared them to 11 healthy controls (five female). Males with BPD had significantly lower serotonin synthesis in corticostriatal sites, including the medial frontal gyrus, anterior cingulate gyrus, superior temporal gyrus and corpus striatum; however, in females with BPD these changes were seen in fewer regions. Soloff et al. (2003a) conducted a study specifically looking for sex differences in central serotonergic function in BPD.
